چکیده
The optimal approach to ablation of persistent AF has not been well-established, and current strategies are only modestly successful. To assess the efficacy additional vein Marshall (VOM) ethanol lateral mitral isthmus in addition standard using a stepwise approach. Sixty -Five consecutive patients undergoing for were included. . All underwent PVI with RF ,left atrial roof confirmed line block, cavotricuspid performed. VOM concomitant annular ( from LA endocardium Coronary sinus as needed) achieve block attempted all patients. Patients follow-up ambulatory monitoring -office telephonic closed health delivery system at least 12 months f/u procedure. A total 38 65 had successful Vom injection. .35 these achieved block. 27 did receive therapy due venous collateralization (29)%, inability cannulate, vessel (29%) identified (19%) target dissection or extravasation (23%.) With an average follow up months, 82% clinical arrhythmia- free survival. In unsuccessful attempt, arrhythmia survival was 58%. off antiarrhythmic drugs, one One patient readmitted pericarditis without effusion managed colchicine. is associated improved arrhythmia-free ablation.

Catheter Ablation of the Superolateral Mitral Isthmus Line
METHODS AND RESULTS: The study population comprised 114 patients with perimitral flutter who underwent de novo ablation of an MIL. The initial 57 patients (group A) underwent catheter ablation using a novel superolateral MIL design, connecting the left-sided pulmonary veins with the mitral annulus along the posterior base of the left atrial appendage visualized by selective angiography. The nex...
متن کاملA Review of Mitral Isthmus Ablation
Mitral isthmus ablation forms part of the electrophysiologist's armoury in the catheter ablation treatment of atrial fibrillation. It is well recognised however, that mitral isthmus ablation is technically challenging and incomplete ablation may be pro-arrhythmic, leading some to question its role. This article first reviews the evidence for the use of adjunctive mitral isthmus ablation and its...
متن کاملMitral isthmus ablation: the importance of epicardial connections between the coronary sinus and Marshall bundle.
The ligament of Marshall is an epicardial vestigial fold that contains the vein of Marshall and a myocardial sleeve called the Marshall bundle (MB). If the MB epicardial connections bypassing endocardial mitral isthmus (MI) are present, it should be blocked to achieve a complete MI block. We report the first case to create the MB conduction block by disconnecting the coronary sinus–MB connections.

Great teaching value can come from showcasing an unusual phenomenon that serves as an aid to explain how best to approach mundane, common problems. In this installment of teaching rounds, Wakabayashi et al use an unusual activation sequence observed along the mitral annular atrial myocardium that was observed after ablation of the mitral isthmus to teach us about the pitfalls created by the com...
متن کاملMitral Isthmus Ablation: Is the Conduction Block Completed? The Importance of the Marshall Bundle Epicardial Connections.
A 66-year-old man underwent electrophysiological study and catheter ablation for persistent atrial fibrillation. At the beginning of the procedure, sinus rhythm was restored by internal cardioversion. Circumferential pulmonary vein isolation was performed, and the electric isolation of all 4 pulmonary veins was confirmed. Subsequently, linear ablation of a left atrial roof and the mitral isthmu...
